Passport Services

The U.S. Department of State Office of Passport Services has designated Oakley Civic Center as an authorized passport acceptance office.

Everything you need for your passport application is available at the Civic Center located at 3231 Main Street!

Our services feature application processing, same-day passport photo service at a low $15 fee.  To make an appointment between 9am and 4pm for passport services, please contact our receptionist at (925) 625-7000.

When applying for a passport at the City Clerk's office, please have all forms completed (Do NOT Sign the Application Form) and bring required identification and proof of citizenship.  Accepted documents are listed below .

 

HOW TO APPLY IN PERSON FOR A U.S. PASSPORT

You must apply in person for a U.S. passport if any of the following statements describes you:

  • You are applying for a U.S. passport for the first time.
  • Your previous U.S. passport was lost or stolen.
  • Your previous U.S. passport has expired and was issued more than 15 years ago.

SPECIAL REQUIREMENTS FOR CHILDREN UNDER AGE 16

AS PROVIDED BY THE CODE OF FEDERAL REGULATIONS TITLE 22, SECTION 51.28 Effective February 1, 2008.

IMPORTANT: All minors must appear in person when applying for a passport

Both parents, or child's legal guardian must: 

  • Submit evidence of the child's US citizenship and
  • Submit evidence that they are the parents or guardian and
  • Present valid personal identification and
  • submit a photocopy of the front and back of the identification shown and
  • Sign and take the oath before an authorized passport acceptance agent

If the second parent is not available to sign, the appearing parent must:

 

Complete the above and

Present evidence of 

  • Sole legal custody of the child or
  • Notarized written consent of the other parent for the issuance of the passport along with a photocopy of the non-appearing parent's identification or
  • Statement of Exigent/Special Family Circumstances DS-5525 Form Used by the appearing parent/guardian when the notarized, written consent of the non-appearing parent/guardian cannot be obtained or
  • A written statement (made under penalty of perjury) explaining the second parent's unavailability

AS DIRECTED BY REGULATION 22 CFR 51 (Effective February 1, 2004): Each minor child applying for a passport must appear in person

HOW TO APPLY IN PERSON FOR A U.S. PASSPORT

1. Complete an Application Form

Application forms can be obtained at the Civic Center or from the Internet at http://www.travel.state.gov. Please do not sign the application form until instructed to do so.

2. Present Proof of U.S. Citizenship (One of the following)

  • Previous U.S. Passport (if under 16 last passport issuance, you must also provide your certified birth certificate)
  • Certified birth certificate issued by the city, county, or state. (A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, and the date the certificate was filed with the registrar's office.) Online orders can be submitted on the Internet at http://www.vitalchek.com.
  • Consular Report of Birth Abroad
  • Naturalization Certificate
  • Certificate of Citizenship

3. Proof of Identity (One of these)

  • Previous U.S. Passport
  • Naturalization Certificate
  • Certificate of Citizenship
  • Current, Valid Driver's License
  • Government ID: City, State or Federal
  • Military ID: Military and Dependents
  • Work ID: Must Be Currently Employed by the Company
  • Student ID: Must Be Currently Enrolled
  • Merchant Marines Card: Also Known as a "Seamen's" or "Z" Card
    Pilot or Flight Attendant ID

4. One Passport Photograph
    
Photograph must be 2x2 inches in size and taken within the past six (6) months. The image size from the bottom of the chin to the top of the head should be between 1 inch and 1 3/8 inches. The photograph may be in color or black and white. It must be a full face, front view, with a plain white or off-white background. Photograph should be taken in normal street attire, without a hat or headgear that obscures the hair or hairline. Uniforms should not be worn in photograph with the exception of religious attire that is worn daily. If prescription glasses, a hearing device, wig, or similar articles are normally and consistently worn, they should be worn when the picture is taken. Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless required for medical reasons. A medical certificate may be required to support the wearing of such glasses in photographs. Vending machine photos are not generally acceptable.

5. The Applicable Fee

Application Fees: (This fee must be paid by check or money order.)

Payable to:  U.S. Passport Services

16 and older:        

  • $110 - Passport Book (valid for travel by air, sea or land)
  • $30 - Passport Card (valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only)

Under 16 years:   

  • $80 - Passport Book (valid for travel by air, sea or land)
  • $15 - Passport Card (valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only)

Execution Fee: (This fee is paid separately from the application fee and can be paid with check or money order)

Payable to:  City of Oakley

  • $25.00 (per application)

If you need your passport in less than five (5) weeks (25 business days) and you are applying to a passport agency, or in less than six (6) weeks (30 business days) and are applying at an acceptance facility, you will need to pay an additional $60 expedite fee to ensure urgent handling.
